Refill Requests

Fill in blanks below and click SEND EMAIL (at bottom) to request a refill for supplements or  prescription drugs.  If you do not know if you should continue or change your dosage, please email Dr. Cowan first.  Kindly help us avoid confusion by not duplicating your request through voice mail. 

Please let us know how many bottles of each supplement you would like. For Standard Process, include the count and/or the code number if you have it. For drug prescription renewals, leave the referral number for your medicine or the dosage, count, number of refills, along with your pharmacy's fax or phone number.
