The on-line form is for your convenience. It is not secure. If this concerns you, other options include fax, mail, or email of the  printed form. Please just be sure that it makes it to us before your appointment, or you may bring it with you if you have an office visit. For the online form, fill in blanks below, then click SEND EMAIL (if you don’t send it we won’t get it!).  If you want to sign up for the Community Supported Healthcare Plan, please also fill out the CSH Form when you are finished. We are also accepting donations for our Medicine Support Fund for seriously ill patients if you care to participate.  ***If you have concerns about supplement charges, please ask Dr. Cowan during your first appointment.***
